Medication Refill Request Form

To request a refill of any of your pet’s medications, please provide the following information. Upon receiving a prescription refill request, an invoice will be emailed. Prescription processing requires at least two hours. The invoice is payable online. Once payment is confirmed, a follow-up email is sent to confirm that the prescription will be ready for pick-up no sooner than two hours. Instructions for pick-up are provided in the same email.
